Please read and follow these steps carefully to be fully registered.  IMPORTANT:  You are only registered once ALL steps are completed.  You will only sign ONCE during the entire process and that will suffice for ALL documents required. 		  
Step 1: All students who plan to participate in athletics must have a physical examination every other school year.  Physical examinations must be performed by a licensed physician, physician's assistant or advanced nurse practioner.  Athletes WILL NOT be allowed to participate without having a physical exam in good standing.

A physical examination taken April 1 and thereafter is valid for the following two school years.  Physical examination taken before April 1 is valid only for remainder of that school year and the following year.  Therefore, for the 2019-20 school year if your last physical was prior to April 1, 2018, you need a physical. 

If you do not need a physical this year, you do need a signed alternate year card.  This requires only the athlete and parent signature.

All documention should be submitted to the Athletic Office.

Step 2:   Read the Amery High School Co-Curricular Code  and WIAA Eligibility Info at the back of the athletic code carefully. Your one time signature will indicate that you have read this and agreed to abide by what is stated in these documents.

Step 3:  Fill out and submit the Online Registration Form here (includes all forms in one).

Step 4:  Go to Activities Registration Above and Fill out the Emergency Card.  Finally, pay your Athletic Fee.  High School - $100 per sport/Middle School - $50 per sport.  There is a $200 cap per family (can combine middle and high school students).  Athletic fees are waived for those who qualify for Free and Reduced Lunch. Athletic fees must be paid in person, not online.  It may be cash or personal check and can be dropped off at the Amery High School or Middle School office for both middle and high school athletes.
If you were out for a fall and winter sport or paid the family fee you have most likely met the cap.

Once you complete these steps, you are registered and can fully participate in the Amery High School or Middle School athletic program.

 
You may sign up for all of your sports seasons at one time this year, click on all sports you plan to participate in.

IMPORTANT NOTE ABOUT IMPACT CONCUSSION TESTING:

Testing needs to be done yearly, once you have taken a baseline test after April 1st you are good for the next year.  See Mr. Gould or Mrs. Robarge if you need to baseline.
